Paresthesia Caused by Ossification of Ligamentum Flavum at the Thoracic Spine

Abstract
There was a 60 years old man with progressive radiculopathy, paresthesia and weakness on bilateral legs for 1 year. We found central canal stenosis with disc protrusion at L2-5 and ossification of ligamentum flavum (OLF) at T7-12 in his MRI. We performed lumbar epidural block for three times and L4 nerve root block for differentiating the origin of pain. However there was just about 20% improvement of his symptoms. We determined that his symptoms were caused by OLF at the thoracic spine and transferred him to a neurosurgeon. After decompressive laminectomy of T7 and T9-10, his radiculopathy and paresthesia disappeared. Discomfort on lower leg is commonly caused by spinal lesion at lower lumbar level. But, OLF at thoracic spine can cause the similar symptoms with lumbar spinal stenosis. Therefore, we keep in mind that sometimes it can be needed to evaluate other spinal levels for the refractory lower extremity pain.
